• sore throat
• achiness
• respiratiory issues

3. Have you recently had a fever of 100.4 or greater and/or have you or your immediate family been exposed to a known case of COVID-19?
4. Have you traveled outside of the country in the last 30 days?

*** If they are having symptoms and no travel they will need to self-quarantine until they are better.

If they are having symptoms and their travel evaluation i_s positive they need to contact the CDC.
